Although oral tetracycline medications (such as doxycycline) are extremely valuable in the management of patients with hordeolum, lid disease in the presence of rosacea, and non-responding ocular surface disease, these agents often cause several undesirable side effects.
Prescribe low dosages of doxycycline (e.g., 20mg). Research from the DEWS report suggests that patients who take lower doses of doxycycline may experience the same anti-inflammatory effect as patients who take 100mg-doses, but with significantly fewer side effects.¹ Instruct your patients on b.i.d. dosing not to take the second doxycycline pill before going to bed or immediately lying down. There have been reports of localized irritation if the pill remains in the upper GI or esophagus.² Instead, ask your patients to take their second pill at least two to three hours before bedtime. Because the tetracycline medications are highly acidic to begin with, tell your patients not to take the pills with acidic beverages, such as orange juice, apple juice, coffee, etc. I base this suggestion upon the experiences of one of my patients who had significant gastrointestinal issues after taking her morning pill, but not her evening pill. The patient told me that she usually took her morning pill with orange juice and her evening pill with water. When she switched to water at both times, her stomach problems ceased. Instruct your patients to take doxycycline with food to decrease the incidence of stomach-related side effects. Now, this suggestion may sound confusing because the pharmacy instructions clearly state that these pills should NOT be taken with food. The primary reason for this labeling is because the ingestion of dairy products immediately before or after dosing can render doxycycline completely ineffective. So, inform your patients that they can safely take doxycycline with food if they do not consume a significant volume of dairy products. (Otherwise, patients should take doxycycline at least one hour before or two hours after eating dairy products.)

Proper Use – Drug information provided by: Merative, Micromedex ® Take this medicine exactly as directed by your doctor. Do not take more of it, do not take it more often, and do not take it for a longer time than your doctor ordered. To do so may increase the chance of side effects.
It may be taken with food or milk, if it upsets your stomach. You may open the capsule and sprinkle the contents on a spoonful of cold, soft applesauce. Do not lose any pellets when transferring the contents. The mixture should be swallowed right away without chewing and followed with a glass of cool water to ensure complete swallowing of the pellets. Do not store the mixture for later use.
If you are using the delayed-release tablets or tablets:
It may be taken with food or milk, if it upsets your stomach. You may take this medicine by breaking the tablets. Hold the tablet between your thumb and index fingers close to the appropriate score (separation) line. Then, apply enough pressure to snap the tablet segments apart. Do not use the tablet if it does not break on the scored lines. You may also take this medicine by sprinkling the equally broken tablets onto cold, soft food, such as applesauce. This mixture must be swallowed right away without chewing and followed with a glass of cool water to ensure complete swallowing of the pellets. Do not store the mixture for later use.
Swallow the capsule whole. Do not break, crush, chew, or open it. You may take the Acticlate® Cap capsules with food or milk to avoid stomach irritation. Shake the oral liquid well just before each use. Measure the dose with a marked measuring spoon, oral syringe, or medicine cup.
- Drink plenty of fluids to avoid throat irritation and ulceration.
- If you are using this medicine to prevent malaria while traveling, start taking the medicine 1 or 2 days before you travel.
- Take the medicine every day during your trip and continue taking it for 4 weeks after you return.
- However, do not use the medicine for longer than 4 months.
If you are using Oracea™ delayed-release capsules:
You should take this medicine on an empty stomach, preferably at least 1 hour before or 2 hours after meals. You should take this medicine with a full glass of water while sitting or standing. To prevent throat irritation, do not lay down right after taking this medicine.
Use only the brand of this medicine that your doctor prescribed. Different brands may not work the same way. To switch from Doryx® delayed-release tablets to Doryx® MPC delayed-release tablets:
Replace a 50 milligram (mg) dose of Doryx® delayed-release tablet with a 60 mg dose of Doryx® MPC delayed-release tablet. Replace a 100 mg dose of Doryx® delayed-release tablet with a 120 mg dose of Doryx® MPC delayed-release tablet.
To help clear up your infection completely, keep taking this medicine for the full time of treatment, even if you begin to feel better after a few days. If you stop taking this medicine too soon, your symptoms may return.
What makes doxycycline less effective?
Before taking doxycycline, –
tell your doctor and pharmacist if you are allergic to doxycycline, minocycline (Dynacin, Minocin, Solodyn, Ximino), tetracycline (Achromycin V, in Pylera), demeclocycline, any other medications, sulfites, or any of the ingredients in doxycycline capsules, tablets, extended-release tablets, or suspension. Ask your pharmacist for a list of the ingredients.tell your doctor and pharmacist what prescription and nonprescription medications, vitamins, and nutritional supplements you are taking or plan to take. Be sure to mention any of the following: acitretin; anticoagulants (‘blood thinners’) such as warfarin (Jantoven); bismuth subsalicylate; carbamazepine (Epitol, Tegretol, others); isotretinoin (Absorica, Clavaris, Myorisan, Zenatane); penicillin; phenobarbital; phenytoin (Phenytek); and proton pump inhibitors such as dexlansoprazole (Dexilant), esomeprazole (Nexium, in Vimovo), lansoprazole (Prevacid), omeprazole (Prilosec, Zegerid, in Talicia), pantoprazole (Protonix), and rabeprazole (Aciphex). Taking doxycycline does not give you full protection against malaria.you should know that when doxycycline is used during pregnancy or in babies or children up to 8 years of age, it can cause the teeth to become permanently stained and can cause problems with bone growth. Doxycycline should not be used in children under 8 years of age except for inhalational anthrax, Rocky Mountain spotted fever, or if your doctor decides it is needed.
Unless your doctor tells you otherwise, continue your normal diet. Take the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
Why can’t you drink milk with doxycycline?

Antacids and dietary supplements containing calcium may also have similar effects.
- You should avoid taking these products at the same time that you take your doxycycline prescription.
- You don’t have to completely remove dairy from your diet.
- Just pay attention when you’re consuming dairy products.
- It’s recommended to avoid consuming dairy products within 2–3 hours of taking doxycycline (Patel, 2022).
For example, if you choose to take doxycycline at 8 a.m., you’d want to avoid eating dairy from about 5 a.m.–11 a.m. A few common dairy foods you’ll want to avoid around the time you’re taking tetracycline antibiotics include:
- Milk
- Buttermilk
- Butter
- Cream and heavy cream
- Ice cream
- Cheese
- Cream cheese
- Cottage cheese
- Kefir
- Yogurt and frozen yogurt
- Whey
- Desserts made with dairy products
- Ghee
Can doxycycline cause liver damage?
High dose intravenous doxycycline can cause acute fatty liver typical of that caused by intravenous tetracycline, particularly in susceptible patients such as pregnant women. This type of injury is, however, quite rare.

WHAT IS DOXYCYCLINE? – Doxycycline is an antibiotic used for both its antibacterial as well as its anti-inflammatory properties. For acne and rosacea it is common for this medication to be used continuously for many months. Take with food, preferably immediately before eating.
- This insures that the pill is diluted by food so that it does not irritate your stomach, and that the pill gets all the way down into your stomach.
- Sometimes when it is taken with a small swallow of liquid the pill will lodge in the esophagus, where it is very irritating, and can cause chest pain.
- Do not take Doxycycline immediately before going to bed.
While you are lying down the pill can reflux back up into the esophagus, where it can irritate or even ulcerate the lining of the esophagus. Take it at least 1 hour before bedtime. You do NOT have to avoid milk products when taking this medication. Milk, and anything with calcium or iron, will slightly decrease absorption, but not enough to affect your treatment significantly.
(This differs from the older drug tetracycline.) If you take antacids, laxatives, calcium or iron supplements, try to avoid taking these within 2 hours of doxycycline.
